Generational Care, LLC is a single physician owned and operated primary care office that provides services for patients of all ages. Our mission is to provide compassionate and dedicated healthcare, while breaking down barriers to access and ensuring affordability of medications.
Generational Care, LLC is a medical clinic without a brick-and-mortar office. Instead, office visits will be completed via telemedicine. However, in the future, house calls in the select Northern Kentucky counties of Boone, Kenton, Campbell, and Pendleton will be offered.

New patients for urgent care telehealth visits start at $79 for patients that pay at time of service.
For patients paying at the time of service, urgent care visits for already established patients (which are typically about 15 minutes) start at $50.

For patients paying at the time of service, a standard new patient primary care 30-minute appointment is $79. If the appointment for these patients exceeds 30 minutes, additional time will be charged at $20 per additional 30 minutes.
For patients paying at the time of service, a standard established patient primary care 30-minute appointment is $60. If the appointment for these patients exceeds 30 minutes, additional time will be charged at $15 per additional 30 minutes.
Asynchronous Visits
Need care as an established patient, but can't commit to a live visit due to your schedule or other factors?
Our asynchronous telehealth visits offer a flexible, convenient way to receive medical advice. Simply submit your health concerns, symptoms, or photos (such as of a rash or injury) at your convenience via OnPatient Portal or via theOhMd app that is available on iphones and android phones. Then Dr. Rebecca Osborne Lewis will review your information and provide guidance at a later time - typically within 24 hours.
This option is perfect for non-urgent matters or follow-up care, and allows you to receive expert medical advice on your schedule. Fast, secure, and hassle-free!

For patients paying cash at the time of service, a standard asynchronous visit is $25. This covers a simple visit such as a patient sending a message about mild acne with attached photos and the provider reviews the patient's history and replies with a treatment plan.
​
If the visit is a little more in depth such as a patient messaging about worsening seasonal allergies and the provider reviews the chart about previous treatments, etc and provides medication adjustment, then that visit will be $30 for patients paying cash at the time of service.

If the visit is much more in depth such as a parent messaging about child's ADHD symptom escalation and the provider reviews past behavior notes, school input, adjusts treatment plan and recommends a follow up visit, then this visit would be $45 for patients paying cash at the time of service.

If, upon review, a live telehealth visit is deemed more appropriate for your needs, the cost of the asynchronous visit will be applied toward the fee for the live visit.
No Show Policy:
A no-show fee of $30 will be charged to all appointments that are missed and not cancelled prior to the appointment time, unless prohibited by the patient's insurance provider. If there are extenuating circumstances, then this fee may be waived at the discretion of Generational Care, LLC.
